Tooth numbers and surfaces.
A concern around #31. A restoration on the mesial of #14. Alta Notes is trained on dental terminology, so the numbers and surfaces land in the note the way they were said in the room.
Alta Notes · SOAP notes software for dental practices
Alta Notes is SOAP notes software built for dental practices. It listens to the appointment conversation and drafts a structured note in SOAP or your practice’s own format. Your clinician reviews and edits it, then copies it into the practice management software.
Why dental is different
General clinical note tools are built around the medical visit. The dental appointment has its own vocabulary, its own measurements and its own software, and the note has to carry all three.
A concern around #31. A restoration on the mesial of #14. Alta Notes is trained on dental terminology, so the numbers and surfaces land in the note the way they were said in the room.
Pocket depths, bleeding on probing, suppuration, mobility and recession. With Alta Voice Perio, the perio chart and the 3D Perio Index can be selected as sources for the note.
Dentrix, Eaglesoft, Open Dental, Denticon or Curve. The reviewed note is copied into the patient’s note field in the software your team already uses.
From visit to documentation
Nothing goes into the record until a clinician has read it. You review every note in Alta Notes before you copy it.
Record the conversation in the operatory and choose the sources for the note. The microphones are included, and the speech engine is trained on masks, suction and operatory noise.
Alta Notes writes the note in your template: your headings, your order, your level of detail. Check the output, edit the details and make sure the note reflects the care provided.
Copy the approved text and paste it into the patient’s note field. Your clinician stays responsible for the final clinical documentation.
Inside the note
SOAP is one template. The example below uses demonstration data and the standard four sections. Your practice can keep SOAP or bring its own structure.
Subjective. Chief complaint, pain description, onset and duration, relevant dental history and the patient’s concerns, as the patient described them.
Objective. Extraoral and intraoral exam findings and periodontal findings by tooth, such as bleeding on probing at #7, #8 and #25 through #32, suppuration at #9 and mobility at #9.
Assessment. The provider’s findings and diagnosis as stated in the room.
Plan. Treatment discussed, the options presented, the patient’s decision and the next steps.

Choosing SOAP notes software
Bring these to any demo, including ours.
1. Dental vocabulary. Say tooth numbers, surfaces and perio terms the way you do chairside and read what comes back.
2. The chart handoff. Watch the full path from finished note to your practice software, with your software, not a generic one.
3. Template control. Ask how a provider’s headings, order and level of detail get changed, and who does it.
4. Onboarding. Ask who sets up the operatory, the templates and the team training. Alta Voice gives every practice 1-on-1 onboarding.
5. Security. Confirm HIPAA compliance, a BAA and what happens to the audio. Alta Notes retains no patient audio.
